Insurance
Currently I accept the following insurance plans:
- Blue Cross Blue Shield
- Meridian
- Priority Health
- United Health Care
- Some Medicaid – depending on who handles your managed care
- Tricare (out of network)
Please contact me to verify your insurance is accepted or to learn about out of network and self-pay options.
Payment
I accept cash, check and all major credit cards as forms of payment. I also accept payment directly on my site via this page and IvyPay
Cancellation Policy
I understand that things come up, so you can cancel or reschedule your appointment with at least 24 hour’s notice. If it’s less than 24 hours notice, you will be charged for the appointment unless you have an emergency situation.
